Prior to scheduling your PET or PET/CT scan, your referring physician must fax a requisition form to the Columbia Kreitchman PET Center. Once we receive that form, a PET Center coordinator will contact you to:

• Obtain all necessary patient information, including address, insurance information, and medical history.
• Verify insurance benefits and pre-certification when required.
• Schedule a convenient time and date for your PET or PET/CT scan.
• Provide you with instructions on how to prepare for your scan, as well as detailed directions to the PET Center.

You will be asked to bring films from prior diagnostic studies, such as a CT scan or MRI.
